Major incidents challenge every part of the hospital, but nowhere do they have a more immediate—and more intense—impact than the Emergency Department and Intensive Care Unit.

Using a realistic, mixed adult‑and‑paediatric bus‑crash scenario, we will work through the critical early actions that determine patient outcomes. The aim of the study day is to enhance understanding of the key actions required from ED and ICU nursing staff during a major incident. The programme will explore not only the immediate responsibilities within the department but also provide insight into what unfolds at the scene of the incident, including the coordinated response of the wider emergency services. By understanding the wider system, participants can better anticipate the flow of information and the rationale behind pre‑hospital decisions.

A central focus of the day is preparing teams and clinical areas to safely and efficiently receive a sudden influx of casualties, often with little warning. This includes surge planning, communication pathways, resource allocation, leadership under pressure, and maintaining situational awareness in a rapidly evolving environment. The emphasis is intentionally less on individual clinical interventions and more on the operational and organisational aspects of managing a major incident—ensuring staff feel confident, coordinated, and ready to respond when demand escalates unexpectedly.

From past experience, we’ve found that staff often have limited insight into how a major incident unfolds or into their specific responsibilities in such a scenario. As local training opportunities in this area have largely disappeared, we felt it was important to address this gap through our upcoming study day.
